What: 6th Grade Registration - PROOF OF RESIDENCY MUST BE PRESENTED

Where: SVMS Campus

All incoming 6th graders must provide proof of residency.

Proof consists of the following:
-Utility Bill
-Rental Receipt
-Mortgage Receipt
-Tax Forms

Forms must show parent name and address. No other forms will be accepted.
